Topical Steroids - Best Treatment For Chronic Paronychia

In a double-blind trial, it is found that topical steroids work better than systemic antifungals in treating chronic paronychia (CP).
In the study, 45 adult patients with CP were followed for six weeks after three weeks of treatment with either systemic antifungals (itraconazole or terbinafine) or topical corticosteroid (methylprednisolone aceponate). Efficacy measures included clinical and photographic evaluation.

Topical steroids were more effective than systemic antifungals for the treatment of chronic paronychia (CP), according to the results of a randomized, double-blind trial reported in the July issue of the Journal of the American Academy of Dermatology.
“The involvement of Candida in the pathogenesis of CP has never been proven, even though this condition is commonly considered a type of Candida onychomycosis,” write Antonella Tosti, MD, from the University of Bologna in Italy, and colleagues. “This study shows that topical steroids are more effective than systemic antifungals in the treatment of CP, and supports the view that CP is not a type of onychomycosis but a variety of hand dermatitis caused by environmental exposure.”
In this open, double-dummy study, 45 adult patients with CP were followed for six weeks after three weeks of treatment with either systemic antifungals (itraconazole or terbinafine) or topical corticosteroid (methylprednisolone aceponate).
Of 48 nails treated with methylprednisolone aceponate, 41 were improved or cured at the end of the follow-up period. This result was significantly better than the number of nails improved or cured with terbinafine (30 of 57) or itraconazole (29 of 64).
